DR. KHOOBEHI: First a patient, typically the patients that come with the main complaint that they're unhappy with their shape. For the breast augmentation patients, typically the patient complained of the size not big enough and they want to go bigger. For the breast lift patient, typically is a patient that they're not happy with the shape, and for those patients, one of the key thing is that you want to bring the nipple up, you want to bring tissue up. Some of those patients also are going to need to have some more tissue to, and for that one you combine the lift with the implant. In my practice, I typically like to stage it. I like to do the lift first and come back to the implant, to give better shape, and also it's much safer. Under certain circumstances, I will do the procedure together, but most patients I like to do it in a stage. And lately I've been also doing another procedure, transferring the fat for the upper part of the breast, and that's an option that I offer to my patients that they have sagginess, but also they have deficiency of the breast tissue. When the patient comes for breast lift, there are a couple things that you try to achieve. One is the nipple position, and also some patients come in that have an enlarged nipple areola complex. And during the procedure for the lift you try to make the nipple area complex smaller, and also position the nipple areola in a position above the inframammary fold area. And the other thing is that typically this patient complains about is that they have extra tissue. Most of these patients have been through pregnancy and they had larger breasts in pregnancy. Some of them have done breast feeding and now, after finishing with their child bearing and finishing with breast feeding, they find out that there is just too much extra skin. And for these patients, implant is not going to do the job, because there is too much extra skin. So you have to remove some of the skin and also reposition the nipple areola.
